UK Deaf Sport is thrilled to be working in partnership with Boots Hearingcare to offer free audiograms for deaf athletes. Athletes can book appointments directly with their nearest Boots World of Hearing stores to arrange their sporting audiograms.

Find your nearest Boots World of Hearing Centre and make a booking following the information below.

Guidance to deaf athletes

1. Make your Boots World of Hearing appointment request by email to info@bootshearingcare.co.uk
2. Add Deaf Sport PTA Appt Request in email subject
3. Allow appointments within 3-12 weeks of sending email
4. Choose your nearest or preferred World of Hearing Store – see map above
5. Note in the email your if you have hearing aid, unilateral or bilateral cochlear implant or no equipment.
6. Include in the email your:
   1. Full Name
   2. Date of birth
   3. 1st line of your address and postcode
7. Wait for a reply and confirmation from Boots World of Hearing.
8. Take the ICSD audiogram form with you – downloaded by clicking the button below and filling in the form

When completed return your audiogram to admin@ukds.org.uk
